WSU policy requires all injuries be reported by the injured employee\u2019s supervisor within twenty-four hours. Prompt reporting also eliminates delays in potential benefits to individuals.<\/p>\n\n\n\n<p>For injuries that occur over the weekend or after regular business hours (Monday through Friday, 8 a.m. to 5 p.m.), reports should be submitted the next regular business day after the accident.<\/p>\n\n\n<div class=\"wsu-accordion\">\r\n    <h2 id=\"unique-id-1__12594\" class=\"wsu-accordion__title\">\r\n        <button class=\"wsu-accordion__title-button wsu-accordion--toggle\" aria-expanded=\"false\" aria-controls=\"unique-id-1__content\">Supervisor&#8217;s Responsibility<\/button>\r\n    <\/h2>\r\n    <div id=\"unique-id-1__content\" class=\"wsu-accordion__content\" aria-labelledby=\"unique-id-1__12594\">\r\n        <div class=\"wsu-accordion__content-inner\">\r\n            \n\n<p><strong>Verify<\/strong> circumstances of the incident by interviewing the injured person, witnesses, and other involved individuals.<\/p>\n\n\n\n<p><strong>Report<\/strong> serious injuries to EH&amp;S immediately at 509-335-5251.<\/p>\n\n\n\n<p><strong>Submit<\/strong> incident report.<\/p>\n\n\n\n<p><strong>Complete<\/strong> investigation report if any one of the following occurs:<\/p>\n\n\n\n<ul>\n<li>Employee receives medical treatment.<\/li>\n\n\n\n<li>Employee is unable to work next or subsequent shift(s) due to the incident.<\/li>\n\n\n\n<li>Conditions of significant near miss would have caused serious injury or illness.<\/li>\n<\/ul>\n\n\n\n<p><strong>Send<\/strong> copies of optional personal statement form(s) to HRS, the department\u2019s safety committee, and EH&amp;S.<\/p>\n\n        <\/div>\r\n    <\/div>\r\n<\/div>\n\n<div class=\"wsu-accordion\">\r\n    <h3 id=\"unique-id-1__12731\" class=\"wsu-accordion__title\">\r\n        <button class=\"wsu-accordion__title-button wsu-accordion--toggle\" aria-expanded=\"false\" aria-controls=\"unique-id-1__content\">Injured Person\/Witness<\/button>\r\n    <\/h3>\r\n    <div id=\"unique-id-1__content\" class=\"wsu-accordion__content\" aria-labelledby=\"unique-id-1__12731\">\r\n        <div class=\"wsu-accordion__content-inner\">\r\n            \n\n<p>An optional <a href=\"https:\/\/policies.wsu.edu\/prf\/documents\/2017\/10\/2-26-witness-injured-person-statement.pdf\/\">personal statement<\/a> may be submitted.<\/p>\n\n\n\n<p>Per <a href=\"https:\/\/policies.wsu.edu\/prf\/index\/manuals\/2-00-contents\/2-24-reporting-accidental-injuries-work-related-illnesses\/\">SPPM 2.24<\/a>, the supervisor submits copies of this form (i.e., the witness form) to HRS and references the Incident Report submitted to HRS, the department\u2019s safety committee, and EH&amp;S.<\/p>\n\n        <\/div>\r\n    <\/div>\r\n<\/div>\n\n<div class=\"wsu-accordion\">\r\n    <h2 id=\"unique-id-1__45986\" class=\"wsu-accordion__title\">\r\n        <button class=\"wsu-accordion__title-button wsu-accordion--toggle\" aria-expanded=\"false\" aria-controls=\"unique-id-1__content\">Reportable Incidents<\/button>\r\n    <\/h2>\r\n    <div id=\"unique-id-1__content\" class=\"wsu-accordion__content\" aria-labelledby=\"unique-id-1__45986\">\r\n        <div class=\"wsu-accordion__content-inner\">\r\n            \n\n<h3 class=\"wp-block-heading\">Injury<\/h3>\n\n\n\n<p>Any wound or damage to the body resulting from an instantaneous event. Examples include cuts, bruises, contusions, chipped teeth, amputations, insect bites, electric shock, burns, sprains, and injuries to muscles, joints, and connective tissues resulting from a slip, trip, or fall.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Illness<\/h3>\n\n\n\n<p>A condition resulting from chronic exposure to the work and\/or academic environment. Examples include musculoskeletal disorders (e.g., tendinitis, carpal tunnel syndrome), skin disorders, respiratory conditions, and noise-induced hearing loss.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Significant Near-Miss<\/h3>\n\n\n\n<p>Any incident that occurs that does not result in injury but had the potential to result in injury. An example of this would be an explosion in an empty lab that would have likely resulted in injury had someone been present.<\/p>\n\n        <\/div>\r\n    <\/div>\r\n<\/div>\n\n<div class=\"wsu-accordion\">\r\n    <h2 id=\"unique-id-1__12525\" class=\"wsu-accordion__title\">\r\n        <button class=\"wsu-accordion__title-button wsu-accordion--toggle\" aria-expanded=\"false\" aria-controls=\"unique-id-1__content\">Incident Investigation Report<\/button>\r\n    <\/h2>\r\n    <div id=\"unique-id-1__content\" class=\"wsu-accordion__content\" aria-labelledby=\"unique-id-1__12525\">\r\n        <div class=\"wsu-accordion__content-inner\">\r\n            \n\n<p>The investigation of accidents and significant near misses is an essential part of the University\u2019s accident prevention program. A thorough investigation reconstructing the chain of events leading up to the incident identifies unsafe acts and conditions requiring corrective action. An accident investigation and subsequent implementation of the corrective action minimizes the potential for future accidents.<\/p>\n\n\n\n<h3 class=\"wp-block-heading  wsu-font-size--xmedium\">Supervisors must complete an Incident Investigation Report if <em>one or more<\/em> of the following conditions are met:<\/h3>\n\n\n\n<ul>\n<li>The employee receives medical treatment.<\/li>\n\n\n\n<li>The employee is unable to work the next full or subsequent shift(s) because of the injury or illness.<\/li>\n\n\n\n<li>Events and conditions involving a significant near miss where injury or illness would have been serious.<\/li>\n<\/ul>\n\n\n\n<p>Download the <a href=\"http:\/\/cas.wsu.edu\/safety\/documents\/2023\/03\/supervisors-accident-investigation-report.pdf\">Supervisor&#8217;s Accident Investigation Report<\/a>.<\/p>\n\n\n\n<p>See WSU <a href=\"https:\/\/policies.wsu.edu\/prf\/index\/manuals\/2-00-contents\/2-24-reporting-accidental-injuries-work-related-illnesses\/#form\">Safety Policy and Procedures Manual<\/a> for more information.<\/p>\n\n        <\/div>\r\n    <\/div>\r\n<\/div>\n\n<div class=\"wsu-accordion\">\r\n    <h2 id=\"unique-id-1__67952\" class=\"wsu-accordion__title\">\r\n        <button class=\"wsu-accordion__title-button wsu-accordion--toggle\" aria-expanded=\"false\" aria-controls=\"unique-id-1__content\">Motor Vehicle Accidents<\/button>\r\n    <\/h2>\r\n    <div id=\"unique-id-1__content\" class=\"wsu-accordion__content\" aria-labelledby=\"unique-id-1__67952\">\r\n        <div class=\"wsu-accordion__content-inner\">\r\n            \n\n<p>Regardless of how minor, the driver must immediately notify his or her supervisor and Risk Management Services, as well as the WSU Motor Pool if a Motor Pool vehicle was involved. See <a href=\"https:\/\/policies.wsu.edu\/prf\/index\/manuals\/7-00-motor-vehicle-safety\/7-20-motor-vehicle-accidents\/\">SPPM 7.20<\/a> for full details.<\/p>\n\n\n\n<p>The driver must submit a completed Washington state <a href=\"https:\/\/des.wa.gov\/services\/risk-management\/state-vehicles-and-drivers\/reporting-accident-involving-state-driver\">Vehicle Accident Report<\/a> to Risk Management Services within two working days.<\/p>\n\n\n\n<p>For an accident involving a motor vehicle with injuries, the driver must also immediately notify the local area law enforcement department.
