All nomination packets must include the following materials and be submitted as a single PDF file:

CAS Award Nomination Form

A Word document form is provided for each award. A link to download the appropriate form is available following each award description and from the Nominations Forms webpage.

Nomination Statement

A broad description of why the nominee is deserving of the award. Cite specific examples and focus on the impact of the nominee’s work within the discipline, unit, across the institution, and/or on students as appropriate. List external recognition of note. Limit of two pages, except for the Distinguished Faculty award, which may extend to three pages. See individual award descriptions for details.

Nominee’s Curriculum Vitae or Resume

See individual award description or nomination instructions for page limitations.

Supplemental Materials

Include additional documentation of outstanding or exemplary work; see individual award descriptions for details. Letters of support or testimonials are required.
